Prevotella

Prevotella

Prevotella is a genus of Gram-negative, non-spore-forming rods that are obligate anaerobes. Normally, they colonize various human mucous membranes, but under certain conditions, they cause severe purulent-inflammatory lesions of internal organs and cavities.

MorphologyGram-negative rods with pronounced pleomorphism.
CultivationThey form opaque, pigmented colonies on blood agar.
ResistanceNatural resistance to penicillins and cephalosporins.
SusceptibilityThey instantly die upon contact with atmospheric oxygen and are susceptible to bile.

Taxonomy and ecology

The genus Prevotella belongs to the family Prevotellaceae. The type species and most studied representative is P. melaninogenica. To date, microbiologists know more than twenty different species belonging to this vast group.

Normally, these microorganisms are part of the normal human microflora. Their main biotopes include the oral cavity, colon, and mucous membranes of the urogenital tract. The clinical significance of these bacteria is immense: they frequently act as causative agents of severe purulent-inflammatory pathologies. Most often, they affect the maxillofacial region, abdominal organs, and urogenital structures.

Morphological and Cultural Characteristics

Microscopic examination shows that members of this genus are Gram-negative, rod-shaped bacteria. An important distinguishing feature is their pronounced pleomorphism—the ability to change shape and size. The bacteria are completely non-motile and lack the ability to form spores.

By type of respiration, they are obligate anaerobes. A characteristic diagnostic sign is observed when cultivated on blood agar: Prevotella species form dense, opaque colonies that produce a specific pigment. The color of such colonies can be gray, brown, or even deep dark (black). Biochemically, they exhibit moderately pronounced saccharolytic activity.

Virulence Factors and Pathogenesis

The antigenic structure of the bacteria is represented by somatic O-antigen. An extensive arsenal of pathogenicity factors allows the bacteria to successfully colonize tissues and provoke inflammatory responses:

  • Pili: specialized surface structures providing primary adhesion (attachment) to host cells.
  • Endotoxin: the classic toxic component of the Gram-negative bacterial cell wall.
  • Aggression enzymes: neuraminidase and IgA protease, which actively destroy the protective barriers of mucous membranes and neutralize immunoglobulins.
  • Metabolites: during their metabolic activity, bacteria release volatile fatty acids, which also play a significant role in the development of inflammation.

Resistance to Environmental Factors and Antibiotics

Members of the genus Prevotella are extremely sensitive to physical and chemical environmental factors. Their most important feature is instant death upon the slightest contact with atmospheric air (oxygen). In addition, they exhibit marked susceptibility to bile, which limits their distribution in certain parts of the digestive tract.

From the perspective of clinical pharmacology, it is important to remember their resistance profile. The bacteria possess innate (natural) resistance to penicillin antibiotics and cephalosporins, which must be considered when empirically selecting antibacterial therapy.

Briefly: answers to topical questions

Which Prevotella species, besides P. melaninogenica, have the greatest clinical significance?

In addition to P. melaninogenica, the species Prevotella intermedia has the greatest clinical significance. This microorganism is one of the key pathogens in inflammatory diseases of the periodontium and gingival tissues.

  • Prevotella intermedia (Prevotella intermedia) is associated with the development of acute and chronic periodontitis, as well as specific lesions of the gingival tissues (gingivitis and pericoronitis).

What methods are used for the laboratory diagnostics of infections caused by Prevotella?

For the laboratory diagnosis of Prevotella infections, the bacteriological method is primary, requiring strict adherence to anaerobic techniques during sample collection, transportation, and cultivation.

  • Bacteriological method — inoculation followed by identification based on biochemical and antigenic properties.
  • Bacterioscopic method — has low diagnostic value.
  • Gas-liquid chromatography — used to detect final metabolic products, including volatile fatty acids.

Class and exam questions

What type of respiration is characteristic of Prevotella?

They are strict (obligate) anaerobes. Oxygen is lethal to them—the bacteria die instantly upon contact with air.

Why do Prevotella species frequently cause maxillofacial infections?

The oral cavity is one of the natural biotopes of these microorganisms. When immunity decreases, they cause purulent-inflammatory processes in this area.

How do these bacteria grow on nutrient media?

On blood agar, they form opaque pigmented colonies. The color can vary from gray and brown to deep black.
