Serological diagnostics
Serological methods are actively used for laboratory confirmation of the diagnosis. Their main task is to detect specific antibodies produced by the immune system in response to Pneumocystis jirovecii invasion.
Two main methods are used in modern practice:
- ELISA (enzyme-linked immunosorbent assay);
- IFA (immunofluorescence assay).
A key step is the correct interpretation of the results obtained. If IgM class immunoglobulins are detected in the patient's blood, this clearly indicates an acute infection. An alternative criterion for an acute process is the analysis of paired sera: if the physician records a clear increase in IgG class immunoglobulin titers over time, this also indicates an acute course of the disease.
Principles of etiotropic therapy
The treatment of pneumocystosis requires the prescription of specific antimicrobial agents. The basic drug of choice to suppress the pathogen is co-trimoxazole.
Alternative regimens are used in situations where the primary medication cannot be administered. The following are used as reserve options in clinical practice:
- Pentamidine;
- Caspofungin;
- Combination therapy combining primaquine with clindamycin.
Prevention and control of risk groups
Preventive measures for pneumocystosis have a clear epidemiological focus. Since the pathogen spreads via the aerogenic (airborne droplet) route, all preventive measures must focus on preventing this exact format of transmission.
Special clinical attention should be paid to patients in risk groups. The absolute priority in this category is HIV-infected patients. For them, a crucial component of pneumocystosis prevention is the systemic improvement of their immune status, which helps reduce the likelihood of developing a manifest form of the disease.
Briefly: answers to topical questions
To which kingdom and class of microorganisms does Pneumocystis jirovecii belong?
The pathogen Pneumocystis jirovecii belongs to the phylum Ascomycota (ascomycetes). According to its taxonomic position, this microorganism is classified as an opportunistic yeast-like fungus, but in its morphological features and drug sensitivity, it exhibits properties of protozoa.
What clinical material is used for microscopic diagnosis of pneumocystosis?
For microscopic examination to detect the pathogen Pneumocystis jirovecii, bronchoalveolar lavage (BAL) fluid, sputum, and lung tissue biopsies are used.
What morphological forms does Pneumocystis jirovecii have in the human body?
The life cycle and development of the pathogen occur in the lumen and alveoli of the lungs. It includes four sequential main stages:
- Trophozoite (Trophozoitus) — an oval, amoeboid form 1.5–5 µm in size, covered with a pellicle and capsule.
- Pre-cyst (Praecista) — an intermediate rounding stage with a thickened cell wall.
- Cyst (Cysta) — a form 4–8 µm in size with a thick three-layered shell rich in polysaccharides.
- Intracystic bodies (Corporecula intracystica) — daughter bodies (sporozoites) 1–2 µm in size with a small nucleus and a two-layered shell, forming a "rosette" inside the cyst.
What staining methods are used to detect Pneumocystis cysts in smears?
Microscopic staining methods are used to detect the pathogen and its structure in smears. Romanowsky–Giemsa staining reveals the internal structure, with the cytoplasm staining blue and the nucleus reddish-purple. Special methods are used to identify the cyst cell wall: toluidine blue staining and Gomori–Grocott silver impregnation, which stains the wall polysaccharides.
What clinical symptoms are characteristic of Pneumocystis pneumonia?
The clinical presentation of Pneumocystis pneumonia is characterized by the development of pronounced bronchopulmonary syndrome. Patients experience the following symptoms:
- Persistent dry cough;
- Progressive shortness of breath (dyspnea);
- Fever;
- Feeling of chest tightness;
- Tachypnea;
- Occasionally — production of foamy sputum.
To memorize alternative drugs, use the abbreviation "PCP": Pentamidine, Caspofungin, Primaquine (+clindamycin).
Class and exam questions
Which markers in a serological assay indicate acute pneumocystosis?
An acute infection is indicated either by the initial detection of IgM antibodies or by a dynamic rise in IgG antibody titer in paired sera.
Which drug is prescribed first?
The drug of choice for the treatment of pneumocystosis is co-trimoxazole.
Who is at the highest risk from pneumocystosis?
The main risk group consists of HIV-infected patients with a reduced immune status.
How is the pathogen transmitted?
Pneumocystis jirovecii is characterized by an aerogenic or airborne droplet route of transmission.
