Scope of differential diagnostic search
Differential diagnosis of shigellosis and intestinal amebiasis is carried out with two main groups of gastrointestinal diseases:
- Infectious and parasitic diseases: other protozoan infections, shigellosis, salmonellosis, escherichiosis, yersiniosis, campylobacteriosis, and trichuriasis.
- Non-infectious intestinal pathology: colorectal neoplasms, ulcerative colitis (UC), Crohn's disease, and pseudomembranous colitis.
Note: endoscopic examination methods play a decisive role in differential diagnosis with the second group of diseases.
Clinical and laboratory examination
To confirm the diagnosis and assess the patient's condition in the presence of comorbid conditions, a justified scope of clinical and laboratory investigations is required. The examination plan includes:
- Confirmation of the primary diagnosis using laboratory and instrumental methods.
- Assessment of the general condition of the body and organ functions in the setting of an acute intestinal infection.
- Accounting for severe cardiovascular failure for timely adjustment of therapeutic tactics.
Prognosis assessment
Determining the prognosis in intestinal infections requires an integrated approach:
- Assessment of potential risks to the patient's life and health.
- Account of the severity of the acute intestinal infection.
- Analysis of the impact of pronounced cardiovascular failure on the general course of the pathological process and treatment outcomes.
For the differential diagnosis of gastrointestinal disorders, we divide pathologies into two groups: I&P (infectious and parasitic, such as shigellosis) and NIP (non-infectious, such as ulcerative colitis and neoplasms, where endoscopy is the key).
Class and exam questions
Which disease groups are used to differentiate intestinal amebiasis and shigellosis?
They are differentiated from infectious and parasitic diseases (salmonellosis, yersiniosis, campylobacteriosis, etc.), as well as from non-infectious intestinal pathologies (ulcerative colitis, Crohn's disease, neoplasms, pseudomembranous colitis).
What plays a decisive role in the differential diagnosis with non-infectious intestinal pathology?
Endoscopic examination methods play a decisive role in the differential diagnosis with non-infectious gastrointestinal diseases.
What tasks does the clinical and laboratory examination plan solve?
The examination plan is necessary to confirm the diagnosis and assess the patient's condition in the presence of existing comorbidity.
