Disorders of sexual preference

Paraphilia

Disorders of sexual preference (paraphilias) are a group of deviations in which sexual urge is directed toward non-standard objects or specific actions. In modern classification, they require careful differential diagnosis with variants of normal sexuality, since the boundaries of pathology largely depend on the patient's social maladaptation.

ICD-10 codeIn the classification, they belong to category F65 (includes fetishism, exhibitionism, etc.)
Treatment goalBehavior control and reduction of psychological distress rather than altering the urge itself
GeneticsMale and female homosexuality have a completely different biological nature
Impact on lifeAtypical sexual behavior can critically disrupt a person's adaptation in society

Classification of paraphilias

According to ICD-10, disorders of sexual preference are grouped under category F65. The main forms include:

  • Fetishism (F65.0) — obtaining gratification from viewing certain objects (most commonly items of intimate clothing).
  • Fetishistic transvestism (F65.1) — achieving arousal through dressing in clothes of the opposite sex.
  • Exhibitionism (F65.2) — an irresistible urge to publicly expose one's body.
  • Voyeurism (F65.3) — a pathological tendency to secretly watch people who are naked.
  • Pedophilia (F65.4) — sexual attraction of adults to minors.
  • Sadomasochism (F65.5) — achieving mutual gratification by inflicting mental suffering or physical pain.
  • Other disorders (F65.8) — a group including zoophilia, frotteurism, and making obscene phone calls.

The thin line between norm and pathology

The medical interpretation of sexual disorders is significantly difficult. Diagnosis is influenced by cultural traditions, numerous social restrictions, and public opinion, which is often shaped by the mass media.

It is important to understand that individual elements of fetishism, fetishistic transvestism, and sadomasochism can be variants of normal if they are present in relationships between consenting harmonic partners.

Due to changing social views, homosexuality and bisexuality are today classified as variants of healthy behavior. An exception is ego-dystonic sexual orientation (F66.1). In this condition, the person is extremely dissatisfied with their non-standard orientation, suffers from it, and independently seeks medical help. In addition, psychiatry still lacks clarity regarding the classificatory status of obsessive urges — conditions in which an atypical urge is constantly present in the patient's thoughts but is never implemented in practice.

Treatment approaches

Violations of sexual behavior are characterized by high persistence, so their therapy has specific tasks. The physician does not aim to alter the patient's initial urge itself. Work is structured along the following directions:

  1. Improving control. The patient is trained to manage their behavior so as not to break the law and social norms.
  2. Finding safe forms. Formation of socially acceptable ways to fulfill the existing urge.
  3. Eliminating secondary symptoms. Relief of depression and deep psychological distress.
  4. Removing stigma. Assisting in freeing oneself from self-blame so the patient can openly acknowledge their orientation.
  5. Pharmacological correction. In some severe cases, a general reduction in the intensity of the sexual drive is applied.

Etiology of behavioral deviations

A combination of factors plays a role in the emergence of behavioral and impulse disorders. Genetics provides the foundation — temperament, which only half determines personal diversity. Interestingly, male and female homosexuality have different natures: concordance (matching trait) in monozygotic twins among men is about 50%, whereas in women it is zero.

Psychological schools explain deviations differently:

  • Behaviorists blame improper upbringing (hypo- or overprotection) and failures in the reward and punishment system.
  • Psychoanalysts look for the cause in pathological fixation at early phases of psychosexual development (oral, anal, or phallic).
  • Organic theory emphasizes the influence of mild central nervous system damage in early childhood, which leads to emotional lability and psycho-organic syndrome.

Class and exam questions

Is homosexuality considered a mental disorder?

No. Due to changing public opinion and scientific data, homo- and bisexuality are classified as variants of healthy behavior. Help is required only in cases of ego-dystonic orientation, when the person themselves suffers from their urge.

What is the difference between simple fetishism and fetishistic transvestism?

In fetishism, sexual arousal arises from viewing objects (for example, intimate clothing). Fetishistic transvestism requires actually dressing in clothes of the opposite sex to achieve gratification.

Can paraphilia be completely cured and the object of desire changed?

As a rule, these disorders are very persistent. Therapy is aimed not at changing the urge itself, but at improving behavioral control, relieving depression, and finding socially acceptable forms of expression.
