Ankylosing Spondylitis

*Ankylosing spondylitis*

Bekhterev's disease (ankylosing spondylitis) is a systemic chronic joint disease leading to ossification of the ligamentous apparatus and fusion of the vertebrae. The condition is accompanied by severe pain at rest, the development of a "suppliant's posture", and a high risk of dangerous spinal injuries.

Characteristic posture"Suppliant's posture" with marked spinal kyphosis
X-ray sign"Bamboo spine" sign due to total ossification
Fracture riskThe risk of spinal cord injury is 10+ times higher than in the general population
Pain at restUnlike osteochondrosis, pain increases at rest

Clinical presentation and appearance

The main external manifestation of advanced pathology is the specific "suppliant's posture", characterized by marked thoracic kyphosis. Due to limited mobility and joint fusion, respiratory function suffers drastically: chest expansion decreases, which, combined with bad habits, exacerbates respiratory failure.

Smoking cessation is critically important, as shortness of breath caused by the rigidity of the chest cage is significantly amplified by tobacco smoke.

Radiodiagnostics and radiological signs

Spinal radiographs reveal unique changes that allow for accurate verification of the disease:

  • "Bamboo spine" sign: appears on an anteroposterior radiograph due to total ossification of the ligaments and the spread of syndesmophytes along the entire column.
  • "Tram-track" sign: three vertical shadows in the lumbar region formed by fused joints and spinous processes.
  • The MRI picture may demonstrate concomitant dural ectasia — widening of the spinal canal with scalloping of the posterior surface of the vertebral bodies.

Differential diagnosis with osteochondrosis

The following signs help distinguish ankylosing spondylitis from conventional lumbar osteochondrosis:

  • Pain character: in spondyloarthritis, pain only increases at rest, whereas in osteochondrosis, it subsides.
  • Mobility: in Bekhterev's disease, both chest expansion and lateral bending of the spine are impaired.
  • Laboratory parameters: an increase in ESR and the appearance of markers of acute inflammation are noted.
  • X-ray: signs of sacroiliac joint inflammation and ankylosis are found, which are absent in osteochondrosis.

Complications and rehabilitation

The spine of patients with seronegative spondyloarthropathy is extremely fragile, creating a high risk of fractures in the cervical, cervicothoracic, and thoracolumbar junctions even with minimal trauma. Treatment of such fractures can be either conservative (traction, halo apparatus) or surgical (decompression, arthrodesis, osteotomy to reduce kyphosis).

To maintain musculoskeletal function, gentle low-impact physical exercises, such as Pilates or Tai Chi, are mandatory.

How to memorize

"Bekhterev's in a bamboo and a suppliant's posture: X-rays will show a stick, and pain at rest will not fade."

Class and exam questions

How does spinal pain in Bekhterev's disease differ from osteochondrosis?

In Bekhterev's disease, pain increases at rest, while in osteochondrosis, conversely, it decreases. In addition, in spondyloarthritis, signs of inflammation in blood tests and specific radiological changes of sacroiliitis are detected.

What radiological signs are characteristic of ankylosing spondylitis?

Key signs are the "bamboo spine" sign (total ossification of ligaments) and the "tram-track" sign in the lumbar region, reflecting fusion of the zygapophysial joints and ossification of the interspinous ligaments.

Why are patients with Bekhterev's disease strongly advised to quit smoking?

The disease itself limits chest mobility and causes shortness of breath, and tobacco smoke further exacerbates the developing respiratory failure.
