Endodontic Handpieces and Their Classification
Specialized endodontic handpieces are traditionally used for mechanical root canal preparation. They are used in combination with standard electric or air micromotors of dental units.
The main structural feature of any endodontic handpiece is the mandatory presence of a gear reduction system (reduction gear). The reduction gear is responsible for the critically important decrease in instrument rotation speed. Safe work in the root canal is only possible at low speeds — within 250–350 rpm. Operating the handpiece without a reduction gear for such manipulations is strictly unacceptable.
Depending on the nature of the working file's movement, endodontic handpieces are divided into two groups:
- Reciprocating. They mimic basic manual movements (“filing” and “watch-winding”). They were used with conventional manual endodontic instruments. Today, they are practically out of clinical use due to a complex of serious drawbacks.
- Continuous rotation. In such handpieces, the instrument continuously performs full rotations in one direction. They are created specifically for modern nickel-titanium (Ni-Ti) systems and are necessarily equipped with a torque control system.
Endomotors are the standard of modern practice
The second and most advanced type of drive units are endomotors. These are standalone or built-in devices with an electronic control unit, considered the most optimal option for canal preparation.
Key advantages of endomotors:
- Precise programming. Electronic control allows setting instrument movement modes, using special programs and fine adjustments, which increases work speed.
- Prevention of instrument breakage. The system continuously monitors resistance in the canal. If the torque exceeds the set value, the motor automatically stops and initiates reverse rotation (auto-reverse). This prevents instrument binding.
- Apex locator integration. A number of models have a built-in root apex location function. The device strictly limits the working length of file insertion, preventing accidental periodontal trauma.
Apparatus for three-dimensional canal obturation
After preparation is complete, the canal must be hermetically sealed. Devices for obturation by vertical compaction of warm gutta-percha are classified into a separate group of equipment.
Compaction of gutta-percha cones is carried out using a special heating tip — a plugger. The design of the handpiece allows the plugger to be heated and cooled instantly to a specified temperature. An important feature: heating of the instrument always starts strictly from its tip. This minimizes the risk of thermal trauma to periodontal tissues.
The use of such apparatus shortens treatment time, ensures reliable filling of complex anatomical zones with gutta-percha (including the apical delta and lateral canals), and significantly improves the long-term prognosis of treatment.
Additional equipment in endodontics: scalers and EPT
In addition to specialized motors, other dental devices find application in endodontics:
- Scalers. Devices generating ultrasonic waves (up to 35,000 Hz in piezoelectric models). Although their main function is deposit removal, in endodontics they are used for root canal filling during surgical root-end resection, as well as for the fixation of intracanal posts.
- Electric pulp testing (EPT) apparatus. Used to assess the condition of the dental pulp by applying a low-intensity direct current (2–200 µA). This is the only non-invasive diagnostic method for qualitative disorders in the pulp, indispensable in the differential diagnosis of pulpitis, periodontitis, and dental trauma.
Briefly: answers to topical questions
For what specific purposes are ultrasonic scalers used in endodontics?
Ultrasonic scalers and vibration systems are used in endodontics for root canal preparation, as well as for performing a number of specific procedures.
- Root canal preparation — creating a cavitation effect in the canal lumen with a constant supply of irrigant and cooling.
- Root canal filling — used during root-end resection.
- Placement of structures — use for the fixation of inlays and intracanal posts.
- Working with special instruments — use of Rispi Sonic (similar to a rasp), Shaper Sonic (similar to a barbed broach), and Trio Sonic (three-spiral H-file) tips.
To memorize the basic functionality of an intelligent endomotor, use the “T-A-A” mnemonic: Torque (force control) — Auto-reverse (breakage protection) — Apex locator (working length control).
Class and exam questions
Why is a reduction gear mandatory in endodontic handpieces?
The reduction gear is necessary for a radical decrease in rotation speed. Safe root canal preparation requires low speeds (250–350 rpm), and working with a conventional handpiece without a reduction gear will lead to instrument fracture.
Why are reciprocating handpieces practically unused?
They have a number of significant drawbacks that reduce operational efficiency. They have been replaced by continuous-rotation handpieces and endomotors operating with modern nickel-titanium files.
What is the clinical advantage of directional plugger heating?
The plugger heats up strictly from the tip side, which minimizes the risk of thermal burn and periodontal tissue trauma during warm gutta-percha obturation.
What happens when torque is exceeded in the canal?
When excessive resistance occurs, the endomotor control unit automatically stops file rotation and initiates auto-reverse (reverse rotation) to prevent binding.
