Materials for Temporary Root Canal Filling

Temporary root canal obturation is a critically important stage of endodontic treatment aimed at sanitizing the canal system between appointments. To perform this task effectively, special gutta-percha points with active antibacterial components are used, while calibrated paper points are used for thorough preliminary preparation.

Calcium hydroxide gutta-percha points

This group of materials is intended exclusively for temporary filling, which is mandatory indicated by the manufacturer on the packaging (for temporary filling). The main therapeutic advantage of such points lies in their chemical composition.

Calcium hydroxide (Calciumhydroxid Plus) is integrated into the material matrix. This chemical compound is released gradually, providing prolonged antiseptic action directly within the root canal lumen. Thus, the infectious process is suppressed throughout the entire period the temporary filling remains in the tooth.

For convenience in matching the anatomy of a specific tooth, the points are strictly standardized according to the international ISO system. The classic assortment includes a size range from 15 to 40 (15, 20, 25, 30, 35, 40). Materials are supplied in special plastic cassettes. Inside the cassette, isolated wells are provided for each individual size, and standard packaging often consists of 60 pieces per pack.

Paper points (absorbers)

Before introducing a medicament or filling material into the canal, the operational field must be thoroughly prepared. The main task at this stage is complete drying of the canal lumen and absorption of any residual moisture (exudate, antiseptic solutions). Paper points, or absorbers (Absorbent Paper Points), are used for this purpose.

They are made from high-quality absorbent paper. A key requirement for this class of materials is absolute sterility (sterile). Introducing non-sterile paper into the root canal is a gross violation of asepsis rules in endodontic intervention and is unacceptable in clinical practice.

Markings and size characteristics of paper points

Modern paper points feature well-thought-out ergonomics that significantly speed up the dentist's work and reduce the risk of errors:

  • Color coding (color coded ends). The end parts of the points are colored in strict accordance with the ISO standard palette. This allows the dentist to instantly visually identify the required size. For example, a yellow tip color always indicates ISO size 20.
  • Depth markings (depth marking). Special dark stripes are applied to the lateral surface of the paper cone, acting as a millimeter scale. This marking makes it possible to continuously monitor the depth of instrument insertion into the canal, avoiding going beyond the apex.

In addition to tip diameter (ISO size), points are manufactured with specified taper and total length parameters. For example, points with a taper of .06 and a length of 29mm are widely used in clinical practice. Depending on the clinic's needs, packaging can be either an assortment (different sizes in one cassette) or a set of points of strictly one size (when all wells are filled, for example, only with size 20).

How to memorize

The "Color = Size" rule: a yellow tip of a paper point or gutta-percha point is always ISO size 20.

Class and exam questions

Why is calcium hydroxide added to temporary gutta-percha points?

Calcium hydroxide (Calciumhydroxid Plus) is added to provide a long, prolonged antiseptic effect inside the root canal between patient appointments.

Can non-sterile paper points be used for canal drying?

No, this is strictly prohibited. Paper points are supplied sterile, and maintaining this sterility is critical for compliance with asepsis rules during endodontic treatment.

How can the dentist control the insertion depth of a paper point?

To control the insertion depth, modern paper points feature depth marking — dark stripes forming a millimeter scale directly on the surface of the point.
