Assessment of temporary disability for childcare

A sick-leave certificate for caring for a sick child is issued to a relative or guardian who is actually providing the care. The timelines for issuing the document and the rules for referral to a medical commission strictly depend on the patient's age and clinical prognosis.

Rules for issuing a sick-leave certificate

The issuance of a sick-leave certificate (SLC) is carried out to a family member, relative, or legal guardian. The main condition is that this person must actually care for the patient. The period for which the attending physician has the right to issue the document is strictly tied to the patient's age:

  • Under 7 years old. The document is opened for the entire duration of the acute illness (or for the period of relief of an exacerbation of a chronic process).
  • From 7 to 15 years old. The base period is up to 15 days for each individual case of illness. If the child requires longer care, the issue of extension is decided exclusively by a medical commission (MC).
  • Over 15 years old. A physician can issue work release for a maximum of 3 days. Further extension also goes strictly through the medical commission.

Referral for medical and social expertise (MSE)

If the disease leads to persistent limitations in life activity requiring state protection, the child is referred for medical and social expertise (MSE) to establish the category of "disabled child". The decision on this is made by the medical commission.

The referral timelines depend on the clinical and occupational prognosis:

  1. Unfavorable prognosis. Documents are submitted for expertise no later than 4 months from the moment temporary disability begins, regardless of the current sick leave duration.
  2. Favorable prognosis (protracted course). If recovery prospects are good but the illness is protracted, the referral deadline arrives when the duration of disability exceeds 10 months. In exceptional cases (e.g., tuberculosis treatment, severe trauma, reconstructive surgery), this period is extended up to 12 months.
  3. Working individuals with disabilities. They are referred for MSE regardless of their current disability group and the duration of sick leave if their prognosis has worsened and a correction of the vocational rehabilitation program is required.

Assessment of disease consequences and social guarantees

The regulatory framework for determining limitations and establishing the disability category is the Order of the Ministry of Health and Social Development of the Russian Federation No. 535 (dated August 22, 2005). The very concept of expert assessment is based on the analysis of three interrelated levels of pathology:

  • Impairments. Assessed strictly at the level of a specific organ or anatomical system.
  • Activity limitations. Assessed at the level of the organism as a whole (how capable the patient is of moving, caring for themselves, studying).
  • Social disability. Assessed at the personality level (how impaired a person's integration into society is).

The care system relies on social support (a legislatively established system of measures to ensure basic guarantees) and social protection (comprehensive state measures of an economic, legal, and social nature of a long-term character).

Comprehensive medical and social care

The main goal of any medical and social measures is to compensate for activity limitations and help the child integrate into society. For this, a multidisciplinary approach is used, combining medical, psychological, and socio-pedagogical aspects.

The main implementation tool is the individual rehabilitation program (IRP) for a person with a disability, which is supplemented by state protection measures. Successful social orientation is impossible within the framework of a single agency, so the process requires strict coordination of medical organizations, social protection bodies, government structures, and the participants themselves (physicians, social workers, patients).

How to memorize

The age step for sick leave: "All – 15 – 3". Up to 7 years given for ALL days of illness, from 7 to 15 years — up to 15 days, over 15 years — up to 3 days (further only through the commission).

Class and exam questions

To whom exactly is the childcare sick leave certificate issued?

The document is issued to a relative or guardian who is actually providing care for the sick child, rather than simply being listed as a family member.

When are patients referred for MSE in case of a protracted course of the disease, but with a favorable prognosis?

If disability lasts more than 10 months. In cases of trauma, reconstructive interventions, or tuberculosis, this period can be extended up to 12 months.

At what levels are the consequences of the disease assessed during expertise?

At three levels: organ (impairments), the organism as a whole (activity limitations), and personality (social disability).
