Primary hemostasis pathology
Disorders of the vascular-platelet link include acute immune thrombocytopenia.
- Clinical presentation: The leading symptom is petechial (microcirculatory) type of bleeding. Fine punctate hemorrhages (petechiae) occur due to functional or quantitative failure of platelets in capillaries.
- Laboratory markers: The main diagnostic criterion is prolonged bleeding time. This parameter directly reflects the inability of platelets to timely form a primary white thrombus at the site of vascular wall injury.
- Quantitative criterion: The diagnostic threshold for thrombocytopenia is set at less than 150×10⁹/L. At the same time, it is important to consider the normal platelet lifespan, which is 7–10 days in the peripheral bloodstream. This time parameter is critical for calculating platelet concentrate transfusion intervals.
Secondary hemostasis pathology
Disorders of the coagulation link are associated with a deficiency of plasma clotting factors. A classic example of such pathology is hemophilia.
- Laboratory diagnostics: The key sign is prolonged blood clotting time.
- Unlike conditions associated with thrombocytopenia, the bleeding time in hemophilia remains within normal limits. This is because primary hemostasis in such patients is not impaired, and the primary plug forms adequately.
Patient management basics
The management of children with hemorrhagic syndromes is based on current clinical guidelines for pediatric hematology.
- Coagulopathies: Specific protocols for the diagnosis and treatment of hemophilia are applied, and international classifications of von Willebrand disease (von Willebrand disease) are taken into account.
- Platelet anomalies: The management of immune thrombocytopenia is regulated by specialized expert panels. Rare hereditary forms, such as macrothrombocytopenia — Fechtner syndrome (Fechtner syndrome), require special attention.
- As part of the differential search, the clinician must also keep in mind the existence of hemoblastoses. This is a broad heterogeneous group of malignant neoplasms whose common feature is their origin from mutated hematopoietic cells.
Class and exam questions
How to differentiate primary from secondary hemostasis pathology laboratory-wise?
In primary link defects, bleeding time is prolonged because the formation of the primary white thrombus is impaired. In secondary link failure (hemophilia), bleeding time remains normal, but prolonged blood clotting time is diagnosed.
At what platelet count value is the diagnosis of thrombocytopenia considered valid?
Thrombocytopenia is officially diagnosed when the platelet count in a blood test drops below 150×10⁹/L.
Why does a clinician need to know the platelet lifespan?
A lifespan of 7–10 days is fundamentally important for understanding the mechanisms of thrombocytopenia development and the accurate calculation of platelet concentrate transfusion intervals for patients.
