Essence and physiological principles of the method
Biofeedback is a modern tool of neuropsychological rehabilitation. The main goal of this technique is to help the patient develop sustained skills in deep psychological and muscular relaxation.
During the session, specialized equipment continuously reads the patient's physiological parameters and feeds them back in the form of clear visual or auditory signals. According to clinical guidelines, the key parameters monitored during this therapy are:
- Heart rate (allows assessment of autonomic tone).
- Electromyography (EMG) of the scalp and neck-collar zone muscles (reflects the degree of muscle tension).
Officially, the procedure is classified as electromyographic biofeedback training for central nervous system and brain disorders.
Application in headache treatment
BFB therapy has established itself as a powerful method in neurological practice, especially in the treatment of cephalalgia.
- Chronic migraine (CM). The method is officially recommended as an effective non-pharmacological treatment and serves as an excellent alternative to pharmacotherapy. It is used for long-term individual prophylaxis of attacks. Important clinical note: the effectiveness of hardware-based sessions increases manifold when supported by regular home relaxation training. The evidence base here is at its highest: the recommendation conviction level (RCL) is A, and the evidence reliability level (ERL) is 2.
- Tension-type headache (TTH). Training with EMG monitoring of the scalp and neck muscles is indicated for both frequent episodic and chronic forms of TTH. In this case, the method has an RCL of B and an ERL of 2.
Role in supportive therapy and oncological rehabilitation
In addition to treating pain syndromes, biofeedback is actively integrated into general rehabilitation programs, including for patients undergoing chemotherapy.
In this direction, the method addresses several physiological and psychological tasks simultaneously:
- Cognitive function recovery: training helps restore previous working memory capacity and improve attention span.
- Correction of psychoemotional background: there is a pronounced decrease in anxiety levels.
- Autonomic regulation: respiratory function improves and heart rate variability normalizes.
To achieve a lasting therapeutic effect in rehabilitation, a systematic approach is recommended. The standard regimen includes from 20 to 30 sessions (RCL — B, ERL — 3).
For quick memorization of the main indications for BFB therapy, use the abbreviation MCT: Migraine (and tension-type headache), Cognitive functions (their restoration), Tnxiety (anxiety reduction).
Class and exam questions
What physiological parameters are monitored during BFB therapy?
First and foremost, heart rate is recorded, and electromyography (EMG) of the neck and scalp muscles is performed to assess the degree of muscle tension.
How to increase the effectiveness of the method in chronic migraine?
Clinical guidelines emphasize that the results of BFB therapy improve significantly if the patient additionally performs home relaxation training.
What is the evidence base for the use of BFB in migraine?
The method has a high level of recommendation conviction — A (evidence reliability level — 2). It is recognized as an effective non-pharmacological alternative for long-term attack prophylaxis.
What is the essence of the method for tension-type headache?
In frequent episodic and chronic forms of TTH, the essence of the method boils down to neuropsychological training using electromyography. The patient learns to control and relax the scalp and neck muscles.
