Neurological disorders and paralysis
The paralytic period lasts from several days to two weeks. The main diagnostic sign of this stage is the appearance of paresis and paralysis while sensation remains fully intact. The lower extremities are most commonly affected, with motor disturbances being asymmetrical and uneven, predominantly affecting the proximal parts. Involvement of the cervical and thoracic segments of the spinal cord poses a serious threat of respiratory disorders.
Somatic and visceral complications
In addition to the spinal cord, the pathological process may involve other vital structures and organs:
- Involvement of autonomic centers combined with paralysis of the respiratory muscles leads to severe circulatory disorders and the formation of large pulmonary atelectases.
- Severely ill patients develop interstitial myocarditis.
- Pronounced neurotrophic muscle atrophy develops in the skeletal muscles of the extremities, subsequently leading to persistent contractures and bone deformities.
Long-term consequences and outcomes
During the recovery period, which lasts up to one year, the peak of reinnervation occurs in the first 2–6 months. If the motor neuron pool is completely destroyed, muscle function cannot be restored. In the residual period, patients retain persistent neurological and orthopedic deficits: flaccid paralysis, pronounced muscle atrophy, osteoporosis, and deformities. In early childhood, growth retardation of the affected limb is clearly noticeable. Years later, post-polio syndrome may manifest, characterized by fatigue, fasciculations, and progressive atrophy.
PAD: Paralysis, Atrophy, Respiration (three main directions of involvement in severe poliomyelitis).
Class and exam questions
What is the direct cause of death in severe poliomyelitis?
The main cause of death is paralysis of the respiratory muscles accompanied by circulatory disorders due to involvement of autonomic centers.
What changes occur in the muscles during the acute and residual periods?
The first signs of muscle atrophy manifest on days 10–14. Subsequently, neurotrophic atrophy, contractures, bone deformities develop, and in children — growth retardation of the affected limb.
What is the essence of post-polio syndrome?
This is a delayed complication that occurs years after the disease. It presents as pathological muscle fatigue, fasciculations, and progressive atrophy of various muscle groups.
