Pharmacokinetic profile
Unlike a number of other anthelmintic drugs (e.g., pyrantel and carbendazim), piperazine adipate is characterized by rapid and complete absorption in the gastrointestinal tract after oral administration. Elimination of the drug from the body is carried out primarily via renal excretion. This pharmacokinetic feature requires strict monitoring of the patient's renal function.
Indications for use
The drug is prescribed for intestinal nematodiasis. The main clinical indications are:
- Ascariasis;
- Enterobiasis.
An important feature of the drug is its high safety profile for children: piperazine adipate is officially approved for use in pediatric practice in patients under 1 year of age.
Side effects and contraindications
Adverse reactions may occur during treatment, primarily affecting the nervous and muscular systems. These include:
- Muscle weakness;
- Tremor.
Special instructions: If the patient has renal impairment, drug elimination slows down. This can lead to drug accumulation and the development of severe neurotoxic symptoms.
Contraindications:
- Organic central nervous system (CNS) disorders serve as an absolute contraindication to administration.
- Due to conflicting data on the safety of piperazine derivatives, the general clinical decision is not to prescribe them to pregnant women.
Prescription writing rules
The drug is administered orally. The average therapeutic dose for adults is 1.5–2.0 g per dose.
Example of a prescription for 0.5 g tablets (to administer a 1.5 g dose):
Rp.: Tab. Piperazini adipinatis 0,5 D.t.d. N. 10 S. Take 3 tablets orally per dose.
Piperazine ring in medicinal chemistry (SAR)
The piperazine ring is an important structural element (pharmacophore) not only in anthelmintics, but also in many other pharmacological groups. Knowledge of these structural relationships helps in studying pharmacology:
- Typical antipsychotics: There is a separate group of piperazine phenothiazine derivatives. These include perphenazine, trifluoperazine (contains a methyl radical in the ring), and fluphenazine (contains a hydroxyethyl radical). Esterification of fluphenazine with capric acid allows for the creation of a depot formulation with a duration of effect of 1–2 weeks.
- Fluoroquinolones: The presence of the piperazine ring at position 7 of the quinolone core (combined with a fluorine atom at position 6) provides high antibacterial activity for this group.
- Antifilarial agents: Diethylcarbamazine is also a piperazine derivative and serves as the drug of choice for lymphatic filariasis.
PIPERAZINE: P — Kidneys (excreted by them; neurotoxicity in chronic kidney disease), P — Fully absorbed, P — Pediatrics (approved up to 1 year of age).
Class and exam questions
What is the main pharmacokinetic difference between piperazine adipate and pyrantel?
Unlike pyrantel, piperazine adipate is rapidly and almost completely absorbed in the gastrointestinal tract upon oral administration.
Why is the drug potentially dangerous in renal failure?
The drug is excreted by the kidneys. When their function is reduced, it accumulates in the body, which significantly increases the risk of neurotoxic events.
Can piperazine derivatives be prescribed to pregnant women?
No, the general rule is not to prescribe these drugs during pregnancy, as data on their safety are contradictory.
In the structure of which other drugs is the piperazine ring found?
It is part of certain antipsychotics (fluphenazine, trifluoperazine), antibacterial fluoroquinolones, and the antifilarial agent diethylcarbamazine.
