Mechanism of Action and Structural Features
Nebivolol has a unique chemical structure. It is a racemic mixture consisting of two enantiomers, each determining its own part of the pharmacodynamics.
- d-isomer is a highly selective $\beta_1$-adrenoceptor blocker. Its affinity for myocardial $\beta_1$-receptors exceeds its affinity for $\beta_2$-receptors by 300 times, making it an absolute record holder for cardioselectivity among analogs.
- L-isomer is responsible for a fundamentally different effect — it actively stimulates the production of endothelial nitric oxide (NO).
In addition to this, the drug increases the synthesis of prostacyclin. The combination of these effects produces pronounced vasodilation, which, combined with the classic adrenergic blocking action, ensures reliable blood pressure lowering.
Pleiotropic Effects and Metabolic Profile
Pleiotropic (additional) effects distinguish this drug favorably from older representatives of the group, such as propranolol. First of all, this concerns the metabolic profile. It is ideal for patients with concomitant endocrine disorders.
- Carbohydrate metabolism: the drug significantly reduces insulin resistance and improves glucose utilization by peripheral tissues, which is critically important in type 2 diabetes mellitus.
- Lipid metabolism: against the background of therapy, positive shifts are observed — the concentration of triglycerides decreases, while the level of antiatherogenic high-density lipoproteins (HDL), on the contrary, increases.
In addition to metabolic benefits, the drug exhibits pronounced antioxidant activity and inhibits platelet aggregation and adhesion, providing an antithrombotic effect.
Clinical application and safety profile
In clinical practice, the drug is prescribed for the treatment of arterial hypertension, as well as stable angina pectoris. In angina, an additional advantage is the ability to dilate coronary vessels precisely through the NO-release mechanism. It is also used in the combination therapy of mild to moderate chronic heart failure (CHF).
A convenient dosing regimen — once daily — improves patient adherence to treatment. Due to its highest cardioselectivity, adverse events characteristic of non-selective blockers (e.g., bronchospasm) are minimally developed.
However, the physician must remember that cardioselectivity is an unstable phenomenon. Selectivity of action decreases with an increase in the standard dose from 5 mg to 10 mg per day, as well as in individuals with certain metabolic characteristics.
Metabolism Genetics (CYP2D6)
The pharmacokinetics of the drug are tightly linked to the patient's genetic characteristics, specifically to the polymorphism of the hepatic cytochrome isoenzyme CYP2D6.
- In «extensive» metabolizers (high enzyme activity), the drug undergoes intensive presystemic metabolism during its first pass through the liver. Because of this, its bioavailability is only about 12%, and the half-life ($T_{1/2}$) is approximately 10 hours.
- In «poor» metabolizers the picture is different: the enzyme works poorly, so bioavailability reaches almost a full 96%, and $T_{1/2}$ increases to 30–50 hours.
In poor metabolizers, the concentration of the active substance in the blood is significantly higher, which naturally leads to a substantial drop in cardioselectivity. Due to pronounced interindividual variability, it is extremely important to consider the simultaneous use of other drugs affecting CYP2D6 activity.
Briefly: answers to topical questions
What side effects occur most frequently with nebivolol?
An exact list of the most frequent side effects of nebivolol is not provided in the sources. The following adverse reactions and features are described:
- Bradycardia — it is indicated for nebivolol that it causes bradycardia.
- Bronchospasm — adverse events characteristic of non-selective blockers are minimally expressed with the use of nebivolol.
Very common and common side effects may differ among different drugs of the same class.
Which drugs have clinically significant drug interactions with nebivolol?
Sources indicate that nebivolol metabolism depends on CYP2D6 genetic polymorphism and that it is important to consider the concurrent use of drugs affecting CYP2D6 activity.
The letter d (d-isomer) — defends the pulse ($\beta_1$ blockade). The letter L (L-isomer) — lightness in vessels (vasodilator effect via NO production).
Class and exam questions
Why is nebivolol preferred in concomitant diabetes mellitus?
Unlike many classical $\beta$-blockers, it does not impair carbohydrate metabolism. On the contrary, the drug improves tissue glucose utilization and reduces insulin resistance.
How does dosage affect the selectivity of nebivolol?
When increasing the daily dose from 5 mg to 10 mg, the drug begins to lose its high selectivity for $\beta_1$-receptors, which increases the risk of systemic side effects.
Why is it necessary to know a patient's CYP2D6 status when prescribing nebivolol?
In «poor» metabolizers, the half-life is prolonged to 30–50 hours, and bioavailability increases to 96%, leading to excessive drug concentration and a decrease in its cardioselectivity.
