Classification and therapeutic goal
Ivabradine, along with alinidine and falipamil, represents a new pharmacological group — heart rate-lowering agents.
The main therapeutic target when using these drugs is a reliable reduction in myocardial oxygen demand. This effect is achieved exclusively through the development of bradycardia (slowing of the pulse), rather than by lowering blood pressure or depressing cardiac function.
The key advantage of this approach is that the drugs do not exert a negative inotropic effect (myocardial contractility). This property makes ivabradine an especially valuable tool for treating patients in whom ischemia is combined with concomitant heart failure.
Mechanism of action and hemodynamic effects
The site of action of ivabradine is strictly the sinus node cells. The drug does not affect Purkinje fibers, atrial cardiomyocytes, or atrioventricular node cells.
At the molecular level, the molecule acts as a selective blocker of $I_f$ channels (f-channels). The electrophysiological result of this blockade is the prolongation (lengthening) of the spontaneous diastolic depolarization phase. As a result, the sinus node generates impulses less frequently, and bradycardia develops.
Unlike classical beta-blockers or calcium antagonists, ivabradine works in a maximally isolated and specific manner:
- Conduction: is not reduced (the drug does not slow intraatrial, atrioventricular, and intraventricular conduction).
- Contractility: is not depressed (ventricular pumping and contractile function are fully preserved).
- Blood pressure: is not lowered (there is no risk of arterial hypotension).
Mechanism of the antianginal effect
The reduction in heart rate directly changes the ratio of the phases of the cardiac cycle. Against the background of bradycardia, the duration of diastole — the period of cardiac relaxation — naturally increases.
It is during the prolonged diastole that the most effective blood supply to the myocardium itself occurs. Through this mechanism, two important effects are achieved:
- The volume of main coronary blood flow increases.
- Collateral blood flow increases.
As a result, the heart muscle receives more oxygen under conditions of initially reduced demand for it.
Pharmacokinetic profile
The drug is intended for oral administration (not used sublingually, rectally, or parenterally) and is absorbed in the gastrointestinal tract quite rapidly.
- Bioavailability: is about 40%.
- Distribution: maximum plasma concentration ($C_{max}$) is recorded 1.5 hours after administration. Approximately 70% of the active substance binds to plasma proteins.
- Metabolism: occurs in the liver (rather than in blood plasma or kidneys) with the active participation of the cytochrome P-450 3A4 isoenzyme. During biotransformation, an active metabolite is formed — the N-desmethylated derivative.
- Elimination: the half-life ($t_{1/2}$) is short and equals exactly 2 hours (not 6 or 24 hours). The elimination of metabolites is carried out via two pathways: through the kidneys (with urine) and through the intestines (with feces).
Specific side effects
The most typical adverse effect of ivabradine is related to the organ of vision — phosphenes. Patients may note changes in light perception or sudden flashes of light in the field of vision.
This phenomenon is explained by the fact that $I_f$ channels are present not only in the heart but also in retinal structures of the eye. The blockade of these receptors causes visual anomalies.
It is important to note the high safety profile of the drug in this regard:
- Ivabradine does not lead to morphological (structural) changes in blood vessels or the retina itself.
- Phosphenes are completely reversible. As a rule, they resolve spontaneously within 2 months (not weeks or half a year), even if the patient continues therapy.
باختصار: إجابات عن أسئلة حول الموضوع
What are the official indications for the use of ivabradine?
Indications for the use of ivabradine include:
- Chronic heart failure with reduced ejection fraction — symptomatic HFrEF in sinus rhythm and HR ≥ 70 bpm: as part of combination therapy with a β-blocker or in case of intolerance or contraindications to β-blockers.
- Stable angina pectoris — in sinus rhythm and HR > 80 bpm, if there are contraindications to β-blockers or non-dihydropyridine calcium channel blockers.
What are the absolute contraindications to prescribing ivabradine?
Contraindications to prescribing ivabradine include:
- severe renal impairment;
- paroxysmal tachyarrhythmias;
- pregnancy;
- breastfeeding.
With which drug groups is the co-administration of ivabradine contraindicated?
Ivabradine is not recommended to be combined with non-dihydropyridine calcium channel blockers — verapamil and diltiazem — due to the risk of excessive heart rate reduction, pronounced bradycardia, and conduction depression.
Ivabradine works as a "pure brake" for the sinus node: it lowers the pulse (blocks f-channels) but does not touch blood pressure or the force of contractions. The letter "f" in the name of the channels (funny) reminds us of "phosphenes" — amusing flashes of light before the eyes.
أسئلة من المحاضرات والامتحانات
Which enzyme is responsible for the metabolism of ivabradine?
Main metabolism occurs in the liver with the participation of the cytochrome P-450 3A4 isoenzyme, forming an active N-desmethylated metabolite.
Are phosphenes dangerous when taking the medication?
No, they do not lead to structural changes in the retina and are reversible. Visual symptoms usually disappear spontaneously within two months while continuing administration.
